Dr. Leslie Chang Evertson has been serving as lead Dementia Care Specialist nurse practitioner for the UCLA Alzheimer’s and Dementia Care (ADC) program since 2012. She has played an integral role in the program’s development and implementation. She is the lead trainer for the ADC program model of dementia care dissemination effort funded by The John A. Hartford Foundation. She is co-chair of the healthcare system dementia care intervention and software content expert for a PCORI/NIA-funded pragmatic trial of comparative effectiveness of two models of dementia care (health systems-based and community-based) and the effectiveness of both compared to usual care. At UCLA, Dr. Evertson is active in several healthcare system committees including the UCLA Health PRIDE network and Nursing Equity, Diversity, and Inclusion group.
