Project 4: Improving Communication in End-of-Life Care for Nurses and Nursing Assistants in Long-Term Care Facilities
Project 5: The Psychiatric Background Assessment Recommendation - Interprofessional Communication Tool (PBAR-ICT): Improving Communication of a Critical Geropsychiatric Situation (CGS) to Reduce Antipsychotic Medications in Persons with Dementia in Arkansas Primary Care Settings

This session showcases innovative projects that GNPs have created and implemented in such areas as antipsychotic reduction, increasing completion rates for advanced care directives, and other projects that may cross the spectrum of acute to long-term care. Focus of discussion includes innovation and current outcomes achieved.
