Description

Advance care planning is not a one-time discussion. As a person faces health challenges, the conversation deepens from “who will speak for you?” to “how do I make decisions in the face of serious illness?” Types of conversation involved in end-of-life planning will be re- viewed. Examples, definitions, and tools to empower the discussion throughout the course of illness are provided.
