Description
Older adults are especially vulnerable during transitions of care. Advanced practice nurses are uniquely positioned to positively impact the outcomes of vulnerable patients as they move across settings and between providers. Using a case study format, this session identifies models of care, practice initiatives, and resources available to improve quality and safety during the critical transitions of high-risk older adults and their caregivers.

- Describe the importance of unnecessary hospitalizations of nursing home (NH) residents in relation to health care reform initiatives.
- Discuss strategies to reduce unnecessary hospitalizations.
- Review tools developed for a quality improvement program (INTERACT) designed to improve the management of acute changes in condition and reduce unnecessary hospitalizations.
- Explore strategies for efficient and effective implementation of the INTERACT program in clinical practice.
