Deborah Wolff-Baker MSN, ACHPN. FNP-BC, GS-C

Deborah Wolff-Baker works for Northern California Medical Associates in Sonoma County, California. She was initially hired to round on both acute rehab and custodial patients in several skilled nursing facilities. She also developed a home-based primary care practice focusing on the needs of chronically ill older adults as well as those transitioning across healthcare settings. She graduated from Sonoma State University with her BSN and PHN certificate in May 1997, and in 2008 graduated with from Sonoma State University with a master's degree in nursing and family nurse practitioner certificate, completing a rural community clinic management certificate program through the university in 2009 as well. She holds advance certification as a hospice and palliative care nurse (ACHPN) and is certified as a family nurse practitioner. She currently serves as chair of the GAPNA Health Affairs Committee..
