Interest in home-based care (HBC) is growing as is shown to improve access to care, improve the quality of life for patients and their caregivers, lead to better health outcomes, and reduce healthcare costs. This full-day workshop focuses on two separate aspects related to home-based care: clinical evidence-based practice and clinical evidence-based practice, specifically geriatric syndromes, social determinants of health, medication management, caregiver support, and clinical procedures in the home.
