173 - Anemia Management in Older Adults with CKD Stage 3-5

Older adults with chronic kidney disease (CKD) stage 3-5 have a risk for anemia. It is important to identify those at risk and develop an appropriate plan of care. This session presents an evidence-based approach to assessment and implementation of strategies aimed at achieving optimal health outcomes in this high-risk population.
