243 - Heart Failure Bridge Program: A Recipe for Success Using a Skilled Nursing Facility

This session shares the steps taken to implement a heart failure bridge program in a skilled nursing facility to decrease re-admission rates and improve quality of life for elders with heart failure. Preliminary results of the project are shared.
