As the population ages the proportion of the aged who reside in nursing facilities and senior residences will increase. Unfortunately these facilities and emergency rooms traditionally operate independently, causing communication breakdown in both directions. This session will help you learn how a group of key stakeholders in a local community partnered to improve transitions of care of geriatric users of emergency room care.
