As America’s elderly population continues to grow, there will be more demand for clinicians who are prepared to care for older adults with multiple chronic illnesses. Recent advancements in health IT can be used effectively to transform care delivery for older adults and engage them in their own care. This session discusses the current status of health IT, challenges for clinicians, and strategies to overcome these challenges.
