343 - Cognitive Impairment and CKD and Diabetes

Older adults with CKD and/or diabetes have risks for cognitive impairment, making it difficult to follow a complex regimen that is required to improve health outcomes. This session shows how to assess for cognitive impairment and what evidence-based strategies may lead to better outcomes in this population.
