The ability to deliver high-quality, patient-centered care in the community has received increased attention as our population ages and patients live longer with multiple, chronic advanced illnesses. Through case studies, this session reviews the range of palliative care needs and clinical management approaches encountered in a home-based palliative care program that helps to support primary care practices in caring for advanced illness patients at home.
